All participants and attendees of The Fletcher Baseball World Series
must comply with the conditions and rules of Fletcher Baseball

WAIVER, RELEASE AND INDEMNITY - Read Before Entering Field

In consideration of being allowed to participate in any way in playing, observing, and/or participating in any way, which are understood, known and acknowledged to be inherently dangerous activities, as well as take part in training, practicing, competitions, demonstrations, and all related events and activities, I, (Name of Participant), acknowledge, appreciate, and agree that;

Participants and Agents voluntarily assume full responsibility for any and all risk of bodily injury, death or property damage due to the negligence of Fletcher Baseball., or otherwise, while in or on the premises, owned or controlled by Fletcher Baseball, and/or while competing, playing, practicing, officiating in, or working in any manner for the purpose of participating in the activities, or watching others on the premises, or playing any games on the premises and,

The Participants and the Agents have observed the premises and understand that the activities conducted on the premises bear certain inherent risks and dangers to life, limb, and property; and,

The risk of injury from the activities involved is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN. EVEN IF ARISING FROM MY NEGLIGENCE OF THE RELEASES OF OTHERS, AND ASSUME FULL RESPONSIBILITY TO MY PARTICIPATION; and;

The Participants and the Agents also affirm that they have knowledge that the participant is of good sound health, and able to participate in the events mentioned herein without any ill effect. Should first aid or emergency treatment or other services be rendered; I agree to pay for any and all expenses involved if and when Fletcher Baseball deems it necessary to supply or call for emergency or other services rendered for myself or unto the minor participant; and,

I for myself and behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Fletcher Baseball, their officers, owners, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event("releasees") WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

I HAVE READ THIS WAIVER, RELEASE AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS,UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY ENDIJCEMENT.

FOR PARTICIPANTS OF MINORITY AGE

(UNDER AGE 18 AT THE TIME OF REGISTRATION)

My parent or legal guardian and I hereby certify that I am under 18 years old. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and
agree to indemnify the releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided

above, EVEN IF ARISING FROM THE NEGLIGENCE, OF THE RELEASEES, TO THE FULLEST EXTENT PERMITTED BY LAW.
